A) You can get the application for a replacement Social Security card online or at your local Social Security office.

To apply online, you will need to create a my Social Security account. Once you have created an account, you can log in and click on the “Request a Replacement Card” link. You will then be asked to provide some basic information, such as your name, address, and Social Security number. You will also need to upload a copy of a document that proves your identity, such as a driver’s license or passport.

To apply in person, you will need to bring your Social Security card application (Form SS-5) and two forms of identification to your local Social Security office. You can find a list of Social Security offices near you on the Social Security Administration website.

The Social Security Administration allows free card replacements; you are limited to three per year or 10 throughout your life (name changes and other exceptions don’t count).

Here are the documents you need to bring to your local Social Security office to apply for a replacement Social Security card:

• Two forms of identification that show your name, date of birth, and Social Security number. Acceptable forms of identification include:

If you are applying for a replacement Social Security card because your card was stolen, you will also need to file a police report. You can file a police report online or in person at your local police station.

Once you have submitted your application for a replacement Social Security card, it will usually take about 10 to 15 business days to receive your new card in the mail.
